  1. Your Form 1120, 1120S, or 1065 was selected for an office audit.
  2. The IRS sent Letter 3572A along with Form 4564 (Information Document Request), to request that you call to set up an audit appointment. Form 4564 lists the documents you will be required to provide at the audit appointment.
